Primary Dental Co-care Pilot Scheme for Adolescents (PDCC)
Programme Overview
The Primary Dental Co-Care Pilot Scheme for Adolescents is a three-year pilot scheme that serves as an interface with the School Dental Care Service for primary school students by subsidising part of the cost of private oral check-ups for adolescents aged between 13 and 17. The scheme aims to foster the establishment of a long-term partnership between adolescents and dentists in the private sector or non-governmental organisations and to promote the adolescents’ lifelong habit of regular oral check-ups for prevention of dental diseases. The scheme is based on a co-payment model; eligible adolescents can receive subsidised oral check-up once a year.

Eligibility
1. Adolescent aged 13 to 17 (or will be aged 13 in the calendar year the application to enrol is made. For the avoidance of doubt, a person will cease to be eligible once he reaches the age of 18);
2. Holds a valid Hong Kong Identity Card; and
3. Has joined (eHealth).
For eligible person aged below 16, a substitute decision maker (SDM) should register eHealth for him/her.
Co-payment
The Government will provide a partial subsidy of $200 annually. The Scheme Participant is required to pay the co-payment fee determined by the dentist upon enrolment to the Programme.
Application Method
1. Choose a participating dentist from the website and contact the dental clinic directly to book an appointment.
2. Before the appointment, read the “Participant Information Notice” and “Personal Information Collection Statement” from the website. Complete the Application and Consent Form and the Oral Health Questionnaire.
3. Complete registration in eHealth online, or with iAM Smart. (A substitute decision maker (SDM) can register with eHealth online for a child aged under 16.)
4. Bring the completed form, questionnaire and HKID card to the dental clinic on the day of appointment.
